CHECKLIST - COLLEGIAL CIRCLE APPLICANTS

Directions: This checklist will assist applicants in completing a successful proposal. Check off each item as it is completed. Sign and send checklist to the
o The title of the collegial circle makes clear the focus, curriculum or pedagogical area, the grade level(s), etc.
o Names of participants are listed with school, subject specialty and grade
o The total hours add up to 15; dates of each session and times are listed
o The collegial nature of the activity has been clearly explained.
o Any applicable NYS Learning Standards have been listed.
o Outcomes are clearly stated including suggestions for dissemination
o At least one district goal has been selected from those provided
o Check below the one description which best matches the collegial circle.
o Provides direct follow-up to a course or courses
o Focuses on new curriculum: how to teach it, design effective lessons and support colleagues; a consultant may be sought for some sessions
o Focuses on an identified problem that may not be strictly instructional in nature; topics might be bullying, behavioral and social issues, motivation, organization, etc.; solving the problem requires a study group approach to find and implement solutions.
o Examines lessons, perhaps relating to new curriculum, to obtain
feedback and support from colleagues; student work may be used
to assess effectiveness of lessons; group may meet over several months; focus is pedagogical practice.
o None of the above (if this is the best choice, be sure to explain clearly in the application the need for a collegial activity to achieve the goal of the collegial circle)
